Under the 2026 bilateral agreement between Italy and Sri Lanka, eligible Sri Lankan nationals can convert their valid driving license into an Italian Patente di Guida without sitting a theory or practical exam. While the process itself is straightforward, the most common reason for delays and rejections is incorrectly or incompletely filled forms.
This guide walks you through every field of every form you need to complete — what to write, why it matters, and what mistakes to avoid. Read the entire guide before putting pen to paper.
Golden Rule: Always use a blue or black ink pen. Write in BLOCK CAPITALS throughout. Do not use pencil, correction fluid, or typed entries unless the form specifically permits it.
What Is This Form?
The TT 2112 is the primary application form submitted directly to the Italian Department of Motor Vehicles (Ufficio Motorizzazione Civile). This is the official Italian government form that initiates your license conversion request within the Italian system. Although the form contains multiple pages, for the purposes of a Conversione Patente, your main focus is the second page, which contains your personal declaration and application details.
Fill this form out carefully and in full. Leave no mandatory field blank. If a section does not apply to you, draw a clear horizontal line through it rather than leaving it empty — this shows the officer that the field was not accidentally overlooked.

Ufficio Provinciale Write the name of the city (provincia) where you are submitting your application. For example: MILANO, ROMA, TORINO. Use the full Italian city name in capitals. This must match the Motorizzazione office you are physically visiting.

Il sottoscritto — Surname (Cognome) and First Name (Nome) Enter your full name exactly as it appears in your passport. Write your surname first, then your given name(s). Do not use abbreviations or nicknames. If your name contains special characters not found in the Italian alphabet (e.g., certain Sinhala-derived transliterations), write the closest Latin equivalent consistent with your passport.

Anagrafica — Date of Birth, Place of Birth, Citizenship Enter your date of birth in DD/MM/YYYY format. For place of birth, write SRI LANKA (even if born in a specific city — the country name is sufficient unless the form requests the city specifically). For citizenship, write SRI LANKESE.

Codice Fiscale — Italian Tax Code Write your 16-character Italian tax code clearly, placing one character per box. Do not leave gaps or add spaces. The Codice Fiscale is found on your Italian tax code card (tessera sanitaria) or any official Italian document. Errors in the Codice Fiscale are one of the most common causes of rejection, so double-check each character carefully.

Residenza — Current Address Enter your current registered address exactly as it appears in the Comune's records. Write the city name, street name, and house number. You do not need to write "Via" or "V." before the street name — just the street name and number is sufficient. Use your registered address (residenza anagrafica), not a temporary or work address.

Richiesta — Type of Application and License Category From the available options, tick or write Conversione. In the adjacent field for license category, write the category of license you are requesting — for a standard car license, write B. If your Sri Lankan license covers additional categories (e.g., motorcycles or heavy vehicles), check with the Motorizzazione whether those can also be converted under this agreement.
Signature: Sign the TT 2112 only at the counter in front of the Motorizzazione officer, if instructed to do so. In some offices, the officer will ask you to sign in their presence as a witnessed declaration. Signing too early at home is generally acceptable, but follow the officer's instructions on the day.
What Is This Form?
To obtain the mandatory Certificate of Authenticity — without which your Italian Motorizzazione cannot proceed — you must submit a separate application form to the Sri Lankan Embassy in Rome or the Sri Lankan Consulate in Milan, depending on your registered region of residence in Italy.
This form is used by the Embassy or Consulate to contact the Department of Motor Traffic (DMT) in Sri Lanka and verify that your license is genuine, valid, and was issued prior to your Italian residency. Choose the correct office based on where you live — submitting to the wrong office will cause delays.

Personal Details Enter your full name, date of birth, passport number, and contact details exactly as they appear in your passport. Any discrepancy between this form and your passport will require additional documents (such as a birth certificate or sworn translation) to resolve and will significantly delay your application.

Sri Lankan Driving License Details Write your exact license number, the original issue date, and the expiry date as they appear on your physical Sri Lankan license. If your license has been renewed and the number changed, use the most recent number. Ensure the license category is stated clearly (e.g., Category B for cars).

Date of First Residency in Italy This is a critical field. You must state the exact date on which your residency was first registered at your Italian Comune — not the date you first arrived in Italy, and not the date of any subsequent address changes. Find this date on your Certificato di Residenza Storico, which you should obtain from your Comune before starting the application.

Declarations and Signature Read all declarations at the end of the form carefully before signing. By signing, you confirm that all information provided is accurate and complete, and that you accept responsibility for any false statements. Your signature here must match the signature on your Sri Lankan driving license — this is verified by the Embassy officer. If your signatures have changed significantly, bring supporting identification to explain the discrepancy.
Attach clear, complete copies of all documents listed below to your forms before submission. Partial or unclear copies will be returned. It is strongly recommended to organise your documents in the order listed below and use paperclips rather than staples — never fold documents.

Make Extra Copies

Translations

Using the wrong Embassy or Consulate. Submitting your form to the wrong office (Rome vs. Milan) based on your residency region will cause delays. Always check your registered region first.

Writing the date you arrived in Italy instead of the date of first residency registration. These are often different dates. The correct date comes from your Certificato di Residenza Storico — not your memory, visa, or entry stamp.

Submitting expired documents without renewal receipts. An expired Permesso without the renewal receipt will lead to immediate rejection of your application.

Mismatched signatures. The signature on your forms must closely match the one on your Sri Lankan license. Practice your signature before completing the final form if necessary.
Final Signature Check: Before submitting, compare the signature you wrote on the application forms side-by-side with your Sri Lankan driving license signature. Significant differences — particularly if the style or name spelling differs — can cause the Embassy officer to question the authenticity of your documents and may trigger additional verification steps, adding weeks to your processing time.
Reminder: No third-party agencies or individuals are authorised to handle this process on your behalf. Submit your forms and documents only through the official Sri Lankan Embassy in Rome or the Sri Lankan Consulate in Milan.
